Personal Information Management Policy
The purpose of this Policy is to outline what the persons, to whom this Policy applies, may or may not do in the execution of their duties to ensure the protection of the personal information of patients, and other persons and entities (collectively referred to as “data subjects”) in the possession or under the control of Samantha Jane Leeferink Physiotherapy(“the Practice”).

Samantha Jane Leeferink Physiotherapy (SJL Physiotherapy) is a private physiotherapy practice, which is conducted in accordance with the requirements of the Health Professions Act 56 of 1974 and is subject to the authority of the Health Professions Council of South Africa (“HPCSA”). The practitioners are bound by the Ethical Rules issued by the HPCSA, which include the duty to preserve patient confidentiality.
SJL Physiotherapy Record-Keeping Policy
The purpose of this Policy is to outline the periods for how long records, most notably those containing personal information, which have been created by or are otherwise in the possession or under the control of the Practice, are kept, and what happens to those records on expiry of these periods. The Policy is intended to protect the Practice from exposure to legal liability.
